I would go with the Caleta and Michael. For me staying in Old San Juan means old world colonial and the Milano, where I believe you cannot even open the windows, does not fulfill that requirement - even though it migh have all the modern world requirements you'd expect of a hotel in that price class.
As with all vaction rentals, the nonrefunable deposit is only fair. they are not a huge hotel and if you cancel for serious reasons, your insurance will take care of it. Don't expect small business owners, who will provide shelter for you, to cover your whims if you chance your mind.
