If you do not have a car you do not need reservation.
The ferry ffrom San Juan to either island is NOT on service.
That may take months maybe just imagine..., dirving from San Juan to Fajardo is 55 to 75 minutes, depending on traffic.
Ferry get loaded very fast, if you don't get there early you may loose you ride.
