You should read theculebracalendaronline.com and search for a story of a recent birth in Culebra. I think she had the baby at home with help from a midwife. No worries, that seems to be the "in thing" now in PR, give birth at home. I have talked to the locals in Culebra about this (emergency medical treatment) because it was our concern as well, not for us but for their well being. They seem to have a plan under control and it doesn't seem to be a mayor problem flying anyone to the mainland for treatment, even if it is in the middle of the night. Happy travels! J
