Just take a public taxi from the airport. It would have cost us about $60, but due to getting bumped on our flight down, Delta provided a voucher for our driver! We still tipped him.....even though he got lost in Fajardo @ midnight and spent half an hour wandering around trying to get us to the Fajardo Inn. The ride took the expected one hour or so from San Juan. You just walk out on the curb to the taxi kiosk and tell them you need to go to Fajardo. 4.5 hours should be plenty of time.
