Traveling to el yunque and culebra... doctor recommended typhoid vaccine, especially since they are rural areas "off the beaten path".... has there ever been issues people are aware of regarding typhoid in these areas? I cant find any proof of it...

We would like to eat fresh fruit and from local venders, etc.

Culebra (nor El Yunque) are not the jungles of New Guinea (lived there)..

I can't imagine taking any precautions except the packing of sunblock.

Huh, if you check the CDC website, it is actually recommended to anyone traveling to the Caribbean, especially to rural areas. I'm a family doctor who does some travel medicine, and I never would have thought of this for PR, even Culebra. I just got back from culebra and I very much doubt you could get typhoid there. It's spread through food and water contaminated with infected solid human waste.
FYI, the vaccine is usually given as a series of oral tablets. Doctors don't give it in the office and don't make money off it. We're not all money grubbers, a few of us are actually trying to offer what's best for our patients. I think your doc was trying to do the same.
I absolutely would be up to date on your tetanus vaccine which is due every 10 years. Enjoy your trip, I loved it there and hopefully will be heading back next year!
